Health and medications

You must have your child’s doctor fill out the Immunization Form if he or she does not attend a school in the state of Maryland or is home schooled. The form must be completed and signed (by their physician) on file by June 1, 2012. If not on record in the SummerEdge Office before your child’s session begins, admission may be suspended.

If your child requires prescription or over-the-counter medications including sunscreen during SummerEdge, please complete the Medication Authorization Form and make arrangements through the SummerEdge Director or Nurse to dispense as needed. Medication will be administered only when it is in a pharmacy-labeled container with a physician-signed authorization form.
