Instructions for 2010 Application

• Apply online
• Click here to download and print the application.
• Please complete one application per child.
• Specify your morning option(s) and sign up for the afternoon.
• Remember to tally your total cost.
• Please enclose a $50 registration fee.
• Check, Visa or MasterCard are accepted.
• Remember to have your child’s doctor fill out the {immune} if he or she does not attend a school in the state of Maryland.
• If your child requires medications during SummerEdge, please complete the medication authorization form and make arrangements through the SummerEdge Director or Nurse to dispense as needed.
